To apply for a change in membership status, please use the form below that applies to your situation. Requests are confidential, and will be reviewed by the membership committee and the board of directors.

Application for Life Status
Criteria; 45 years of membership with AOA, and a member in good standing at least one year prior to life status nomination.
Application for Special Status
Criteria; must work minimal hours, 16 per week or less.
Application for Special Status Renewal
Criteria; Special status is an annual renewable classification, must reapply at first of each year requested.
Application for Dues Waiver
Criteria; waivers are offered to members who have the inability to pay OOPA/AOA dues due to medical or financial hardship. Dues waivers are an annual renewable classification, and must reapply at first of each year requested.
Note: per AOA bylaws, status changes must be made in the first quarter of the year.
